For Members

If you have been affected by one of the natural disasters listed below and need assistance with your policy, please contact BCBS FEP Vision Customer Service at 1-888-550-2853. Our BCBS FEP Vision Customer Service reps are here to support you with additional resources based on your needs, including:

• Replacement eyewear: If your glasses or contact lenses have been lost or damaged due to a recent natural disaster, BCBS FEP Vision will replace your eyewear. Ask your eye care provider to call Customer Service at 1-888-550-2583, and we will help them with your request.
• Find a provider: If you need to find an in-network eye care provider – especially if you have been displaced due to a natural disaster – call BCBS FEP Vision Customer Service at 1-888-550-2583 or use our Find a Provider tool. Remember, you can visit any provider in the BCBS FEP Vision network – it does not need to be the provider who supplied your original glasses or contacts.
• Member ID card: If you do not have your BCBS FEP Vision member ID card on hand when you visit an in-network provider, simply tell them you have BCBS FEP Vision insurance, and we’ll take care of the rest.

For Eye Care Providers

• Replacement eyewear: If the member resides in an impacted area affected by a recent natural disaster listed below, please call our Order Entry Department at 1-800-77DAVIS (773-2847) to process replacement orders for eyeglasses or contact lenses.
• Authorization: If your office did not dispense the member’s original eyewear, we will issue you a new authorization. Co-pays will not apply to the new order.
• General Provider Support & Provider Directory Updates: If your practice has been affected by a recent natural disaster, and you need assistance in temporarily updating your status in the Provider Directory, please contact BCBS FEP Vision Customer Service at 1-877-235-5316.
