Do I get a member ID card? What if I lose it?

All BCBS FEP Vision identification cards show only the policyholder’s name. When a covered dependent visits a participating provider, the provider will use the ID number from the card along with the patient’s name and date of birth to verify eligibility.

You can download an electronic ID card from the member portal or mobile app. From the mobile app, you can also save your ID card to your wallet.

Note: BCBS FEP Vision identification cards are sent when you initially join the program or as requested. Requests may be submitted by email or telephone.
