High Option members have the option to use their benefit with a participating provider or visit a provider of their choice, pay in full, and file a claim for reimbursement up to the scheduled maximum. Claims can be submitted:
- Electronically at www.bcbsfepvision.com
- Through the Mobile App
- By completing the Direct Reimbursement Claim form and mailing it to the address on the form (a copy of your paid receipt can take the place of the provider’s signature)
Note: BCBS FEP Vision Standard Option members must receive eye care services with a BCBS FEP Vision participating provider. There is no out-of-network reimbursement available with the Standard Option, except under certain circumstances.
